Address Change Form

To change your address, download the address change form, complete it and bring the completed form along with a copy of your identification to the Medicine Hat Co-op Administration office located at #100-3030 13th Ave SE Medicine Hat, AB.

Equity Withdrawal Form

Medicine Hat Co-op members can withdraw their equity if one of three conditions exist:

  1. Qualify for equity withdrawal under Age Bylaw.  Members 65 and over can apply to have any equity they have accumulated over the amount of $200 returned to them.
  2. If a member of Medicine Hat Co-op moves out of the Medicine Hat trading area, they can apply to have their membership closed and their equity returned to them. Requests must be made in writing using the equity withdrawal form and must be received by mail from the members new address. Requests are then sent to the board for approval and payments are mailed. (approximately 90 days from request)
  3. If a member of Medicine Hat Co-op passes away, the equity would be paid to the estate.

Credit Application
